We stayed at Sokkhak Boutique Resort. It's a little out of town but big on hospitality and they make the best Singapore Sling outside of Singapore: http://sokkhak-boutiqueresort.com
We ate at Malis (kind of more than once) - it's that good: http://www.malis-restaurant.com/siemreap/
Coffee at the Little Red Fox Cafe. It is the best in Siem Reap. Hive Cafe is good also for a change of scenery and good all day breakfast. Pick up the Kendal Village brochure and hit all the shops around Hap Guan Street.
Also, don't forget to go to Bodia to pick up beautiful handmade natural bath products: http://www.bodia-spa.com and while you are in that part of town, wander down some of the laneways in and around the old market for some bargains. Handmade goods can be found in the boutiques across the road.
Phare circus is a fun evening out and a great social cause. If you have a few nights in Siem Reap you have to do it: http://pharecircus.org
